I Need this part for my CRATE cabinet!!!!!!

That's called a jack dish, or a jack plate assembly. If you don't mind it not being a plastic Crate part, They make generic metal ones, available at places that sell rack and case hardware. I'm sure it doesn't have any special parts, just a dish with two 1/4" jacks wired in parallel.--->JMS
